SACYHN established five goals that work toward the vision of high quality programs, services and information as close to home as possible:

| • | Identify and create opportunities for positive change in service delivery |
| • | Involve families and service providers in significant ways in planning and service delivery |
| • | Build and sustain inter-regional, cross-sector, and clinical connections |
| • | Develop, improve, and utilize electronic linkages |
Key areas of activity in meeting these goals include:
| • | Knowledge creation or mobilization |
| • | Facilitation of service delivery |
| • | Policy development and implementation |
| • | Communication, and information provision |
Specific actions within key areas of activity generally have an overarching capacity building focus and are geared to one or more of the following levels:
| • | Community at large |
| • | Network as a whole |
| • | Individual organizations within the Network |
| • | Families and providers |
Some actions are designed to provide an immediate service delivery impact for families; others are more focused on long term planning or new service development, and still others relate to the processes and structures necessary to the development and sustainability of a network. These actions are determined by the SACYHN Steering Committee in priority setting discussions that occur annually or as needed.
